What is Smallpox?

Smallpox has been eradicated, but at least two sites, the Centers for Disease Control and Prevention in Atlanta and the Russian State Research Center of Virology and Biotechnology still maintain viable variola. If variola were ever released by an enemy or by terrorists, morbidity and mortality could be considerable.   1. Thirty years ago, smallpox was endemic in 31 countries affecting 15 million people each year (of which 2 million died). Survivors often remained disfigured or blinded for life.

  2. Smallpox has an incubation period of 7-17 days and develops within 2-4 days. During this period, 10% of light-skinned patients develop a severe rash. Mucosal and skin lesions develop on the second day. These lesions shed virus and allow for aerial spreading of viral particles.

  3. The typical lesions start on the face, spread to the forearms and hands, and finally appear on the lower limbs and trunk within about one week.

  4. Several clinical varieties of smallpox are described. 'Ordinary' smallpox, present in 80% of patients, leads to 3% mortality among the vaccinated but 30% among unvaccinated. The most virulent form, hemorrhagic smallpox, is seen in less than 3% of patients. 96% of these patients die, usually before they develop typical pox lesions.
